How to Deduct Health Insurance Premiums as a Self-Employed Business Owner

Turning Healthcare Into a Write-Off

Paying for your own health insurance is one of the toughest costs of working for yourself.

Fortunately, the IRS offers the self-employed health insurance deduction, which can turn qualifying premiums into a valuable tax break.

The Above-the-Line Advantage

This deduction is generally claimed on Schedule 1 of your individual tax return.

It is considered an above-the-line deduction, meaning it can reduce your Adjusted Gross Income, or AGI. You can receive the benefit even if you take the standard deduction instead of itemizing on Schedule A.

That can make it especially useful for freelancers, consultants, and small business owners paying for their own coverage.

Watch the Profit Ceiling

There is an important limit.

Your deduction generally cannot exceed the earned income from the business connected to the health insurance plan.

If your business breaks even or operates at a loss, the health insurance deduction cannot be used to create or increase that business loss.

This is why your bookkeeping needs to be accurate before calculating the deduction.

Avoid the Employer Plan Trap

Eligibility can disappear for certain months if you or your spouse had access to an employer-subsidized health plan.

For example, if your spouse’s employer offered qualifying coverage and you were eligible to join it, you generally cannot claim the self-employed deduction for premiums covering those eligible months.

When you do qualify, the deduction can extend beyond your own medical insurance.

Eligible premiums may include health, dental, and certain other qualifying coverage for you, your spouse, your dependents, and children who were under age 27 at the end of the tax year.

The policy also needs to be properly established under your business or meet the special rules that apply to your business structure. S-Corp owners have additional payroll and reporting requirements, so do not assume simply paying the premium personally is enough.
